We are conducting an online survey to learn about how person centered/directed planning happens for adults with intellectual/developmental disabilities in Ontario. It is being sent to all developmental service agencies in the province. The survey will help us better understand how different agencies have responded to the need for person centered/directed planning. Participating organizations will receive a report of the survey results.
